초록

말레이시아는 급속히 고령화되는 다민족 사회로, 문화적 특성을 반영한 치매 돌봄에 대한 요구가 증가하고 있다. 본 연구는 1940~1980년대 중국 문화권의 음악을 활용한 라이브 음악 기반 회상치료 집단 프로그램이 경도인지저하(경도인지장애 및 경도치매)를 가진 중국계 말레이시아인 대상자와 돌봄제공자 3쌍에게 적용 가능한지를 평가하였다. 중재는 전문 클라리넷 사중주단이 진행한 60분간의 단일 세션으로 구성되었다. 자료는 기초선, 프로그램 종료 1주 후, 그리고 4주 후에 수집하였다. 평가 도구로는 신경정신행동평가 설문지(Neuropsychiatric Inventory Questionnaire), Zarit 돌봄부담 척도-단축형(Zarit Burden Interview-Short), 알츠하이머 돌봄척도(Gain in Alzheimer Care Instrument)를 사용하여 각각 신경정신행동 증상, 돌봄제공자의 부담 및 돌봄을 통해 얻는 긍정적 경험을 평가하였다. 정량적 자료는 신뢰할 수 있는 변화지수(Reliable Change Index)를 이용하여 분석하였으며, 반구조화 면담을 통해 수집한 정성적 자료는 귀납적 주제분석(inductive thematic analysis)을 실시하였다. 세 쌍 가운데 두 쌍에서는 신경정신행동 증상의 개선이 보고되었으며, 한 쌍에서는 돌봄제공자의 부담이 감소하였고, 다른 한 쌍에서는 돌봄을 통해 얻는 긍정적 경험이 증가한 것으로 나타났다. 프로그램의 효과와 관련하여 네 가지 주제가 도출되었다. (1) 기분의 향상과 행복한 기억의 회상, (2) 삶의 질 향상, (3) 문화적 고려사항, (4) 프로그램 구성의 개선. 본 연구는 말레이시아에서 라이브 음악 기반 회상치료 집단 프로그램의 적용을 위한 기초를 마련하였으며, 향후 연구를 위한 방향을 제시하였다.
